Health Screenings 2019 for Kroger Members

Now is the time to start planning for your Health Screening!  

By participating in this screening, members will qualify for a lower weekly premium for benefits during the 2020 plan year. Members only need to complete the health screening (or a Reasonable Accommodation Form if unable to complete a screening). You do not need to meet any targets in order to receive the lower premium!

Screenings need to be completed by September 30, 2019 to qualify for the lower premium.

You have the following options on where to get your screening:

  • Kroger Pharmacy

    • Schedule appointment by calling 1-877-444-9689 or online at www.krogerscreenings.com.    

    • Print your voucher at https://www.hwplan.org/

  • The Little Clinic

  • Primary Care Physician

    • You will need Medical Care Provider Health Screening Authorization Release Form and You are responsible for faxing or mailing the completed form to the Fund office using address/fax number on the form.
