UFCW Local 227 Family Protection AD&D Plan

In insurance, an accidental death and dismemberment policy pays benefits to the beneficiary if the cause of death is an accident. This is a limited form of life insurance which generally also pays if you lose a limb or a function such as sight, hearing or speech in an accident. Policies pay 50% to 100% of the benefit amount for paralysis as a result of an accident.

It is something we all worry about at one time or another—if I am killed or injured what will happen to my family. We want you to know that when times get tough Local 227 is here to help. That is why we provide the UFCW Local 227 Family Protection Plan. The UFCW Local 227 Family Protection Plan is an accidental death and dismemberment insurance policy administered by Farmers and paid for by UFCW Local 227.

To qualify for the UFCW Local 227 Family Protection Plan you must be a member continuously for 6 months, and members on temporary withdrawal due to sick leave will be covered for up to one year. This plan is fully paid for by UFCW Local 227 and it will not be a cost you or your employer. And we promise, no one will show up on your doorstep to sell you more insurance.

Covered injuries include: loss of hands, feet, legs or arms; loss of sight, hearing or speech; or paralysis in your arms or legs. The amount for injuries will be payable to you.

In the event that you are killed or injured—at work or play—a check for up to $5,000 will be issued to your beneficiaries. If you die from an accident and you have not designated a beneficiary with the union, your estate will receive the money. However, if you wish to designate a beneficiary you will need to fill out the beneficiary form below. Please take the time to fill it out so we have them on file and give it to your union steward.
